lighttpd not creating directory?

I'm running lighttpd on my storage nodes and noticing that new
directories aren't being created.

I found this from last year:

Brad ended up saying:
So just assume it's there & upload.  If you get an error, create the
directories, and then re-PUT.

However, I'm noticing that things aren't re-PUT-ing after the error.
I already had the initial "000" directories created from when I
started with Perlbal.

Notice the 403. The "001" directory does not exist.  Shouldn't it be
automatically created?

Anybody have a quick fix? Is this a known bug?
